Player Registration Form

Player Release and Transfer Form  :To be used from the First Monday after Thanksgiving through July 31st.
Releases & Transfers are not considered complete until the District Commissioner has approved the request in the Cal South Online Registration System.

Player Transfer Waiver Form: To be used during the player transfer roster freeze period from August 1st to the first Monday after Thanksgiving.

Youth Medical Play Down Policy & Form:Request for a medical play down exception to allow a Recreational player to play down in age one year if the certain criteria are met for one seasonal year.

Youth Play Up Policy & Form:Request for player to participate on team aged three (3) or more years beyond player’s proper birth date age division.
